BrightStar Post-Op Transitional Care (WA)



BrightStar Care delivers professional and compassionate care in the comfort and familiarity of home. When someone undergoes surgery, it can take time for them to regain their ability to get around on their own and manage their daily activities once again. The Post-Op Transitional Care package provides in-home nurse support and care for a healthy transition and recovery after surgery. BrightStar uses an RN-led, team-oriented approach to optimize health and well-being and reduce the likelihood of hospital readmission.


These services include (but are not limited to): teaching family-care techniques, monitoring and reporting conditions and changes to supervisory RN and/or Physician, help toileting, help bathing/dressing/grooming, medication management, assistance with medical machinery, dressing changes, and wound care.

The Post-Op Transitional Care package gives you and your family peace of mind with a visit from a licensed nurse to evaluate personal safety within the home, teach the patient and/or caregiver how to change dressings and any other necessary care techniques, and any other support or guidance needed to keep recovery on the right track when returning home.

The Details

  • BrightStar Care does not charge or build in costs for transportation to and from the service location. Time period for service begins when care provider arrives for pickup.
  • Caregivers can transport you in their vehicle or your own vehicle (conditions apply).
  • Care provider may be willing to wear surgical/medical masks if requested and provided at time of service.
  • In some cases, BrightStar may request a credit card to be kept on file at the time of scheduling. If so, it will only be charged if services exceed the time purchased via Rally Registry package or to cover ancillary costs to the care provider (gas/mileage costs).

How to Activate

When checking out: Use shipping address as form to list address where service will be utilized. If purchasing as a gift, you can also include the recipients email address and phone number so they are notified of your purchase.


Using your Service: As soon as you place your order, a confirmation code and instructions for scheduling your service will be sent via email. BrightStar Care will also make a courtesy call to walk through scheduling via phone and answer any questions you may have.
